in this picture, ellie is experiencing a ptsd symptom, having hallucinations and flashbacks to joel's death. after knocking over a shovel, the loud noise quickly switches to the scene where joel dies, cutting back to ellie in the barn. she soon has a much longer hallucination of her walking into the room joel dies, finally switching back to her sitting on the floor, screaming. its quite a significant moment as it shows the overall effect of joel's death on her, and how it messes up with her own life.
